As a new patient we would appreciate it if you would take a couple of moments to read the following...
1. Due to the busy nature of this clinic, patients are kindly requested to give 24 hours’ notice of cancellation. Failure to do so will result in the full cost of treatment being charged for the missed appointment unless the slot can be allocated to another patient at short notice.
2. For those of you that are self-funding patients you will be required to pay for your physiotherapy session at the time of your appointment. We are not able to invoice patients at the end of a course of treatment.
3. For those patients that are using private medical insurance, you must have your physiotherapy pre-authorised. Please bring your insurance details along with you to your first appointment. If for any reason your insurance company fail to pay for any of your treatment, it will be your responsibility to cover any outstanding payments.
This policy is issued by Sport and Spinal Physio and sets out how Sport and Spinal physiotherapy uses and protects any information that you give us. We are committed to ensuring that your privacy is protected. Identifiable information will only be used in accordance with our privacy policy. We have put in place suitable physical, electronic and managerial procedures to safeguard and secure the information we collect.
In most circumstances You have the right to request access, correction or deletion of any "personal data" that is held about you. If any of the information you have provided changes, please let us know. Your "personal data" may be processed for a variety of purposes in the course of running our business and offering our service to you. We will always process personal data in an appropriate and lawful manner in line with relevant data protection principles.
The basis on which we process your personal data is that it is necessary for the purpose of medical diagnosis and the provision of health care treatment. We may also process your none sensitive personal data in the pursuance of our "legitimate interests".

Paper versions are stored securely within a locked filing cabinet at Sport and Spinal Physiotherapy.
How long do we hold your information
All other forms of "personal data" will be kept no longer than necessary. If your data is no longer required it will be destroyed. Some factors which will affect how long we retain your personal data include your continued services.
